TSI elevated, restart small dose of PTU?

Hi Elaine,

I am so hopeful from the last labs I posted and your comments that I am in remission. I requested last week that my gp run a TSI lab while I wait to see a new endo. This is the first time they've done the TSI lab because they told me insurance wouldn't cover it.

results 6.2 -I asked for the ref. range and was told above 1.3 is elevated. I've read other graves patients posts about TSI but I'm still confused. Some of the numbers are in the thousands and other people have numbers like 2.8. Do diff. labs have diff. reference ranges for TSI?

Since my TSI is elevated the gp said to start taking PTU again while I wait on an endo. appointment.

How bad is my TSI level?

Should I go back on a small dose of PTU just so my TED doesn't get bad again. Right now my eyes look better and feel better than they have in months but I know 6 weeks from now it could do downhill.

Hi,

Most TSI tests measure % activity. In this test your serum is added to hamster thyroid cells and the change in thyroid hormone production is measured. In the test you had, TSI immunoglobulins are measured, but blocking antibodies can also appear.

Your level is modestly elevated but PTU could cause your thyroid hormone levels to fall too low. Block and replace would be a better option if you were to continue meds. It takes a long time for TSI levels to fall below the cutoff when this testing method is used. The TSI could fall on its own over time as you continue avoiding environmental triggers.

If you continue with PTU until your endo appt, use a very small dose. Best, Elaine
