Welcome to The Q&A with Elaine Moore. Hi Elaine.

I need your help. We are still trying to figure out what's going on with my Mom and at least she has switched to my doctor who I think is great. My Mom is 65 and when she was 40 she was diagnosed with Hashimoto's and put on a low dose of synthroid. For the last 10 years or so she was on 175mg of Synthroid.

A couple years ago the pharmacy messed up her dosage and needless to say she was on a tiny dose for over 4 months which resulted her in getting very ill for over a year, causing her high bp and heart issues she never had before. Once it was realized her doctor put her on a dose of 150mg...we always wondered why he didn't get her back on her original dose. Though she always complained that she didn't feel well, he kept telling her that her levels were "normal"...she also developed fibromyalgia during this time. Needless to say, as I progressed in my knowledge of thyroid disease, I insisted that she switch to my doctor b/c I never felt that her thyroid issues were being addressed.

My doctor agreed that her T3 was too low and that she needed to supplement it, so she also adjusted her synthroid. I don't think she is at the levels she needs yet. She has many hypo symptoms still including extreme fatigue, muscle and joint pain, brain fog, etc. She has also developed over the last six months an issue with her sugar levels. I really feel that it's due to her thyroid levels not being right. She is not overweight at all, eats very healthy and we have no family history of diabetes. Please help me figure out where she should be in regards to her levels and what dose of synthroid and T3 replacement will help get her there. Also, she has always been diagnosed with Hashimoto's but my doctor was the first to run a TSI test and she has had numbers ranging from 193-200. Could she have Graves and not Hashi? She has had ultrasounds that have noted her thyroid has atrophied and she currently has a nodule on it that they are watching. Please help me help her. I really think most, if not all, of her issues stem from her thyroid being so out of whack for all this time. Sorry for the long message.

Hi Holli,

It's possible that your mother had Graves' disease years ago that ran its course and moved into hypothyroidism. Whether this is the case or she had Hashimoto's all along, her thyroid hormone levels probably need to be higher for her symptoms to abate.

It's difficult to say what dose would be optimal but it looks as though she'd benefit from an increase in both Synthroid and Cytomel. A typical increase would be raising Synthroid to 137 or 150 and Cytomel to 7.5 mg. Then, if the follow-up labs show they could be higher and she still has symptoms, the doses could be increased.
