Welcome to The Q&A with Elaine Moore. Hi Elaine, I'll jump right in. On 2-9-12 was admitted from work with AF, chemically converted (that sucked, almost crashed) lab results that evening after chemical conversion showed: TSH-0.03;Free T4-2.89,free T3-12.8;thryoglobulin 3.5,negative antibodies. 5 hour uptake scan performed 3 days later showed 40% uptake. The Endo I'm seeing is highly regarded in our area, but after consuming material since then, I'm a bit concerned with plan for RAI.

Given that I've had a cardiac event I don't know how much wiggle room or waiting time is reasonable. My Endo is in no way rushing RAI. She explained that treating with Methimazole for now, also metoprolol 25mg bid, baby asa, labs in 3 weeks etc, go from there. Now I have developed a rash/hives, great. I also see an allergist.

Your website is just what I've been looking for, sound medical advice. No sugar coating. I'll continue reading through Q & A's for answers. As of now, I'm limited with regard to heart rate, but yoga is helping. I am a bit nervous as to the "other side of Hypo" as I feel good now. Have just ordered your book from Amazon. thanks, Jeannie

Hi Jeannie,

Your labs show that you're hyperthyroid. If your scan didn't show the presence of nodules, your hyperthyroidism may be caused by Graves' disease, but a scan can only suggest and not confirm GD. It would be good to have a TSI antibody test to tell if you have Graves' disease.

Since you had a cardiac event you wouldn't want to have RAI (if you decide to have it) until your thyroid hormone levels fall and become stabilized. RAI is a high risk factor for thyroid storm.

You should be on a starting dose of 20 mg methimazole. Higher doses are no longer recommended since they don't offer additional benefits and are more likely to cause side effects. Rash/hives often occur when the mmi dose is too high. Usually, Benadryl is used to reduce symptoms and the mmi dose is reduced. People who react to mmi typically do not have problems with PTU.

Since you do yoga, you probably follow a healthy lifestyle already. Avoiding environmental triggers such as aspartame and excess dietary iodine in processed food can help, and if you end up not being able to take ATD meds, the herbal tonic Thyrosoothe is very effective.

Don't get rushed into having RAI. Take your time and study your options, including partial thyroidectomy. Since GD is a self-limiting autoimmune disorder, moving into remission with meds is usually a reasonable goal. Best to you and do some research on the use of magnesium for AF. CoQ10 is also essential for people with hyperthyroidism. Best, Elaine
