Welcome to The Q&A with Elaine Moore. I've been on a lose doage of tapazole (10 mg) since November and my results were in the normal range for a while. NOw, they're back eleveated; last month my TSH was 0.08 and this month it is 0.01. I dont understand why and my physican will probably want to increase the dosage or starte up her conversation about getting RAI again and thats out of the question for me. I feel like stopping the medication altogether since it isn't working for me. Can you recommending taking something in place of it since it isnt working? I have some liquid bugleweed and lemon balm at home. I just started taking CQ10, acetyl l carnitine and other vitamins/minerals to help with my symptoms. Im really frustrated and want to get better. Its very scary and stressful when my dr. brings up RAI, she makes me feel that this is the only solution. I know better than that but it scares me because I dont know what take and I know there is something out there that will work for me. This disease is absoutely horrible and has changed my life for the worst! I just want to feel normal again, will that ever happen?

Hi,

TSH is expected to stay low during treatment, often <.01, and a low TSH during treatment doesn't mean you're hyperthyroid. The TSH is lowered by the thyroid antibodies that cause GD. TSH is not supposed to be used to monitor your meds. Your meds are based on your FT4 level, and it should always be at least at mid-range. TSH only rises if the ATD dose is too high or once you're near remission. The goal with meds is to use the lowest dose needed to keep FT4 at least at mid-range and preferably closer to the high end of the reference range regardless of your TSH level. An FT4 that's too low will cause increased thyroid antibody production and this, besides causing a rise in TSH, may put you at risk for eye problems. There's no reason for you to think meds aren't working.

There's a good chance your dose is too high if your doctor is monitoring your TSH level. You won't feel good unless dosing is done the recommended way, based on your FT4 level. I wouldn't use other meds until you see what your FT4 level is. best, elaine
