Welcome to The Q&A with Elaine Moore. Hi Elaine!
The last time I wrote you, you I had weaned off the Levoxyl as my labs were heading towards hyper. (tsh-.06,FT4-1.5,Ft3-3.9.(FT4-.80-1.80) (FT3-2.7-4.2) I was not feeling well and had other tests run (blood tests like I told you and an ultrasound) and everything was fine,except the thyroid ones. I was having a tightness around my ribcage area for about a month at the time and the doc just thought maybe costochondritis. I had this pain as the labs were climbing it seems. The problem is, is what came first the chicken or the egg?
Did the pain have to do with my labs changing? A month before this they were tsh.35,FT4-1.19,FT3-3.4 The doc said stay on 37.5. Did the stress of the pain or whatever made me go slightly hyper again? My bp went up and so did my heart rate.I felt more hyper. I have been better since off the Levoxyl as far as the faster pulse, unless I get stressed, like now, then I get some palps. I went for labs again, per my doc's orders 2 weeks after the very top ones. She thought I was more hyper too I guess from what I said about my heart rate and Bp. NOW they are: tsh-.24 (starting to come up again) FT4-1.19,Ft3-3.1 What the heck? I thought I was more hyper than 2 weeks ago.I realize I stopped the meds for about a week now, so this would do it I guess..I still have the tightness but not half as bad as last month, have some neck pain(always seem to have pain when labs change, either going up or down)worse this time since being on the higher dose of 50 back at the beginning of Oct. thru beginning of Dec. After 3 weeks, had swelling feeling in my wrists,things felt tight,(I wrote you that too) was on that dose 2 months. I went down to 37.5 in Dec thru March, I figured maybe going from 25 to 50 was too much for me. The doc went along with it. Since I went on the higher dose I have not felt well. I know it takes time to adjust but it's been almost 5 months since starting the higher dose. I had been on 25 last year prior to this. I never felt like this on that dose. My labs looked a little hypo but I felt okay.My labs looked about what the last ones I wrote were, except tsh was around 3. If I go back On the replacement, will stress make labs go up again? It seems this is my trigger. It seems this down up, down, up, is not good for someone for sure. I can't seem to stay stable. Do you think a med like Armour would be better or worse for me? I have been off the 25 for a week now, since I weaned, the doctor knows. I don't know what she'll say now but I am almost afraid to start up again. On the generic 50 my labs were:tsh 1.98,FT4 1.72,FT3-3.4. I had the pain in my wrists etc at this lab,felt swollen even though I didn't look it.
I know the labs don't lie but what should my dose be now? It seems I am very sensitive to the med and my labs change on a small dose higher than 25. I think I feel the best with my FT4 around 1.30 and my FT 3 around 3.3 but don't know how to get to this? Is it because even if I have both Graves and Hashi's, my thyroid still kicks in at times and I can still go somewhat hyper? It happened when my Mom died last year but I was only on 25 so labs didn't get to as high as they were 2 weeks ago.I still felt hyper last year too. I can't even recognize the difference anymore it seems, or they change so fast it makes me not know anymore. Stressing over it adds to it. Do you have any suggestions Elaine? Does natural hormone make you feel like this too? Should I go back on the 25 again or wait longer? Thanks again for your insight as to my very weird case!

Hi Brans,

Our thyroid hormone levels change often depending on our diet, general health, stress, climate, activity level, etc. Normally, the pituitary tries to keep our levels fairly stable by secreting more or less TSH. But when we're on replacement hormone, this is another factor.

It sounds as if you still need to be on replacement hormone and you may be having labs too often to tell what's really optimal. The 50 mcg dose seems too high and the 25 mcg works for the most part. A dose of 37.5 mcg is probably the best solution. Because your 50 mcg dose provided a good FT3 level but too high of an FT4 you could ask about trying Armour trying just 1/2 grain at first. It's worth asking about. Best, Elaine
