Hi Elaine
I was hoping you can help explain what my labs might mean. My endo said they mean nothing just keep taking my .137mcg of synthroid and he might increase them. To me, something seems off, at least from a symptom stand point!
TSH 6.160 (range - .450-4.5), T4 10.7 (range 4.5-12.0), T3 Uptake 36 (range 24-39) Free Thyroixine Index 3.9 (range 1.2-4.9), Free T4 1.9 (range 0.82-1.7) Free T3 2.9 (range 2.0-4.4), B12 672 (range 211-946) D3 40 (range 30-100).
I don't feel right at all. I have all kinds of skin problems..ie...rosacea (flushing) acne, hives terribly dry, heart palps, terrible anxiety, am not sleeping well...etc. I am not sure what supplements to take any more. I was graves, rec'd RAI 8 years ago and have been going through all these ups and downs the last 2 years. I am 39...not sure what changed. I know there were a couple days i didn't take my synthroid, and felt better without it. didn't have the flushing or anxiety....
Any thoughts????? thank you so much!

Hi JuMi,
Although your FT4 is slightly elevated, your FT3 level may be too low for your body's needs causing your TSH to be elevated. By 6 years after RAI, most of us need to be on both T4 and T3 replacement hormone, preferably using a glandular extract like Armour. These are your most important results.
The T3 uptake doesn't measure hormone--it measures how well your binding or carrier proteins link with thyroid hormone. The Index is a calculation that sort of estimates how much free T4 you have. This calculation is not as accurate as your direct measurement of FT4.
Your Vitamin B12 level is good, and your vitamin D3 level is on the low side, which is common in people with autoimmune disorders.
Sleep disturbances and anxiety and also palps can occur in both hyperT and hypoT, whereas acne is more commonly seen in hypoT. Dry skin and hives are more often seen in hypoT. Based on your labs and what you're saying you're having symptoms from too much FT4 and not enough FT3. When this happens, the best solution is switching to a glandular extract. You'd probably need around 2 grains of Armour, but usually patients are started on a lower dose so they can get used to the med and then the dose is increased.
Dr. Ridha Arem describes this situation well in his book The Thyroid Solution when he describes what happened when synthetic levothyroxine was introduced. People who had done well on Armour found they were often having the same problem you described when they were put on levothyroxine alone. Best, Elaine
